With the House Call program, we come to you once a year — right in the comfort of your own home. It’s a convenient, easy way for you to stay on top of your health.

You’ll be contacted by a licensed clinician to schedule your annual house call.

Keep in mind, the House Call program doesn’t replace annual checkups or follow-ups, so you should still see your primary care provider for routine visits. Remember, a house call is available for eligible members at no additional cost.

What can I expect in a house call?

A licensed clinician will provide services such as:

A health and wellness exam

Height, weight, and vital checks

A hypertension screening

A medication and supplement review

Coordination of additional care

Clinical support for early diagnoses

Help with follow-up care

After the visit, you’ll receive a personal health summary, which can be shared with your primary care provider. The summary includes any test results, lifestyle recommendations, and other helpful Medicare resources.
